The patient is a 76-year-old female who presented with a chief complaint of sudden facial deviation to the right, drooling, difficulty closing her left eyelid, and a tight sensation in the left facial muscles upon waking up one week ago. She visited her family doctor and was diagnosed with left-sided facial nerve palsy, for which she received medication. Subsequently, upon a friend’s recommendation, she sought treatment at my clinic.
Examination:
The patient appeared distressed, with her mouth deviating to the right, difficulty puffing out her left cheek, and inability to fully close her left upper eyelid. Multiple tender and sensitive points were found on the left cheek, mouth corner, nasolabial groove, temple, and posterior mastoid region. She also experienced constipation, anxiety, and poor sleep. Her tongue was red with a thin yellow coating, and her pulse was wiry and slippery.
Treatment:
- SNAN Acupuncture: Superficial needling of tender and sensitive areas on the facial skin and fascia. After the procedure, the patient reported an immediate relief of facial tightness on the left side and was very pleased.
- Routine acupuncture: Needling facial acupoints with a retention time of 20 minutes.
- Herbal medicine prescription: A combination of Da Chai Hu Tang and Xiao Chai Hu Tang to dispel wind, clear heat, disperse cold, regulate internal-external harmony, and support proper qi while eliminating pathogenic factors.
Follow-up (after two days):
The patient reported having a daily bowel movement, a pulsating sensation in the left facial muscles without tightness, improved sleep, and a better mood. She tolerated the herbal medicine well without any side effects. Her facial paralysis had significantly improved—her upper eyelid could close, and she could puff her cheek. The treatment plan remained unchanged.
After five treatment sessions, the facial paralysis was completely resolved.
Discussion:
Facial paralysis, caused by facial nerve palsy, has various etiologies, with the most common being prolonged stress, anxiety, weakened immune function, and viral infections. Western medicine primarily treats it with corticosteroids and antiviral medications. Acupuncture is often highly effective, especially when initiated early. If treatment is delayed for 2-3 months, the efficacy decreases. Therefore, early intervention is crucial for the best outcomes.

患者女性,76岁,主诉:一个星期前早晨醒来,突然脸颊向右侧歪斜,流口水,左眼睑闭合困难,左侧颜面部皮肤肌肉发紧,至家庭医生处,诊断为左侧面神经麻痹,给予相关药物治疗。后经朋友介绍,到我诊所治疗。查:患者表情痛苦,嘴角向右侧歪斜,左侧鼓腮困难,左侧上眼睑闭合不能,在左侧面颊部、口角、鼻唇沟、颞部、颈后乳突部等有许多压痛敏感点,同时伴有便秘,焦虑,睡眠差等,舌红苔薄黄,脉弦滑。治疗:1)浅刺解结针法点刺面颊部压痛敏感肌肤和筋膜,点刺结束后,患者感觉左侧面部紧张感消失,非常开心;2)常规针刺面部针灸穴位,留针20分钟;3)中药处方:大柴胡汤,小柴胡汤,祛风清热散寒,通府调和表里,扶正祛邪。两天后复诊,病人诉大便日行一次,左侧面部肌肉感觉在跳动,没有紧张感,睡眠好转,心情好转,服用中药,没有任何副作用,左侧面瘫明显好转,上眼睑能闭合,可以鼓腮。治疗:与前相同。5次治疗后,面瘫痊愈。
按语:面瘫是面神经麻痹因所致,原因很多,但最常见的是因长期紧张焦虑,免疫系统功能低下,因病毒感染所致。西医治疗多以激素抗感染为主。针灸治疗常有好的效果,但必须是早期治疗,效果最好。如果发病后2-3个月再做治疗,效果降低。所以面瘫病人应该早作治疗,以取得最好的效果。浅刺解结针法治疗面瘫病人具有更好的效果,因为其独特的针刺可以尽快消除面部的紧张和压痛,消除炎性反应,促进面瘫部位的血液循环。同时配合中药,调整机体免疫力,恢复面神经活性,在最短的时间内取得最佳治疗效果。
